We will need a copy of each for our records… along with a copy of your pup’s current shot records. We require Rabies, Distemper/Parvo (DHLPP), and Bordetella (Kennel Cough). All must be current during your pup’s stay here at Two Peas. All forms can be emailed to TWOPEASANDAPAW@GMAIL.COM or handed to a staff member during check-in.
